Lease End Equipment Returns

Return Information for Equipment Coming Off Lease

Prior to the physical return of equipment from lease, please make sure that you have read and adhered to the termination guidelines as outlined by your Master Lease Agreement.

Termination notices can be sent to the Lease End Logistics department via mail,(Forsythe Technology Inc. 7770 Frontage Road, Skokie, IL 60077),  via email to or via fax to (847) 213-8340.

Packing and Freight charges are usually paid by Forsythe’s customer—no matter who makes the shipping arrangements.  Please see your Master Lease Agreement for details.

Please know that any equipment returned in non-working condition—whether due to improper use or to improper packing—will be subject to additional fees payable to Forsythe.

Return Information Forsythe Will Need in Order to Schedule Freight

Forsythe’s Lease End Logistic Coordinators will happily arrange packing and shipping of equipment that is returning at the end of a lease. In order to ensure that Forsythe is able to perform these services, the following information must be provided for each return:

  • Address where the equipment is physically located.
  • Name and phone number of a contact person at your site who will help handle equipment details (this information will also be shared with the freight carrier).
  • Name and number of a back up contact person.
  • Information on any security clearance required at your site.
  • Dock information:

Can you receive a large truck?
Does the truck need to have a lift-gate?
               Dock hours
Are there any stairs or special flooring involved?

  • How many pieces of equipment are/will be ready?
  • Will the equipment be packed or, will you need Forsythe to arrange packing through the freight carrier?
  • To what address and attention should the freight invoice be sent?
